docs(wiki): Ralph starts from Session Options only

The welcome screen's "Start Ralph Loop" button is gone (chore/remove-ralph-start-button), and it was the wizard's only entry point, so the page no longer offers the wizard as a second way in. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
