Codeman maintainer 3072449c94 docs(mcp): forward CODEMAN_MUX and CODEMAN_SESSION_ID in the Codex config (#615 merge-time fix) ...

Codex starts stdio MCP servers with a minimal environment, so `codeman mcp`
under the documented Codex config never learned the session it runs in: the
self-protection and the children rule did not apply. Both Codex TOML snippets
now forward the two variables with `env_vars`.
